
What information should be included in a name change petition?
Answer: You will generally be required to provide the following information:
- county of residence;
- date and location of birth;
- reason for changing your name and/or correcting your birth certificate
29-8-102. Petition.
The application to change the name or to correct an error in a birth certificate shall be by petition, in writing, signed by the applicant and verified by affidavit, stating that the applicant is a resident of the county, and giving the applicant's reasons for desiring the change or correction.
Can individuals with criminal records file name change petitions?
Answer: Individuals, who have been convicted of first or second degree murder, or any offense requiring registration as a sex offender, may not file a name change petition in Tennessee.
